Introduction:Rubellaisaherpesvirus.Generallyrubellaisconsideredamildadolescentdisease.However,amaternalinfectioncanbetransmittedthroughtheplacentatothefetus,causingcongenitalrubella.Congenitalrubellamayresultinchroniccardiacdisease,growthretardationhepatosplenomegaly,malformationsandothersevereanomalies.Childrenbornasymptomaticmaydeveloptheseabnormalitieslaterinlife.Toreducetheriskofsuchseverecomplications,accurateserologicalmethodsmustbeperformedtodeterminetheserologicstatusofchildbearing-agedwomen.Thepresenceofrubella-specificIgGinthebloodstreamattestsimmunitytorubella.Awomantestedtobenon-immunecanbeeducatedoninfectionanddifferentiaterubellafromotherexanthematousdiseases.Expectingwomenwithcurrentrubellainfectionshouldbecounseledontheconsequencesofcongenitalinfection.
Principleofthetest:PurifiedRubellaantigeniscoatedonthesurfaceofmicrowells.Dilutedserumisaddedtothewells,andtheRubellaIgG-specificantibody,ifpresent,bindstotheantigenduringincubation.Afterwashingthewellstoremoveunboundsample,antibodytohumanIgGconjugatedwithhorserADIshperoxidase(HRP)isaddedandincubatedat37Cfor30minutes.Unboundconjugateisremovedbyasubsequentwashingstep.AsolutionofTMBReagentisthenaddedtothemicrowells.Theenzymeconjugatecatalyticreactionisstoppedataspecifictime.TheintensityofthecolorgeneratedisproportionaltotheamountofIgG-specificantibodyinthesample.Theresultsarereadbyamicrowellreadercomparedinaparallelmannerwithcalibratorsandcontrols.
LimitationoftheTest:1.ReliableandreproducIBLeresultswillbeobtainedwhentheassayprocedureiscarriedoutwithacompleteunderstandingofthepackageinsertinstructionsandwithadherencetogoodlaboratorypractice.
2.Thewashprocedureiscritical.Insufficientwashingwillresultinpoorprecisionandfalselyelevatedabsorbancereadings.
3.Serumsamplesdemonstratinggrosslipemia,grosshemolysis,orturbidityshouldnotbeusedwiththistest.
