In-vitrodeterminationofInsulinandC-Peptidelevelhelpindifferentialdiagnosisofliverdisease,acromegaly,Cusingsyndrome,familialglucoseintolerance,Insulinimia,renalfailure,ingestionofaccidentaloralhypoglycaemicdrugsorC-peptideinducedfactitioushypoglycaemia.
NewlydiagnoseddiabetespatientoftengettheirC-peptidelevelsmeasured,tofindiftheyhavetype1diabetesortype2diabetes.Thepancreasofpatientswithtype1diabetesisunabletoproduceinsulinandtheywillthereforeusuallyhaveadecreasedlevelofC-peptide,whileC-peptidelevelsintype2patientsisnormalorhigherthannormal.MeasuringC-peptideinpatientsinjectinginsulincanhelptodeterminehowmuchoftheirownnaturalinsulinthesepatientsarestillproducing.
C-peptideassaysmaybeanalyticallymoresensitivethaninsulinassays.MeasurementoftheC-peptidemaybeusefulinevaluatingendogenousinsulinsecretioninavarietyofclinicalconditions.InsulinandC-Peptidearesecretedintoportalcirculationinequimolarconcentrations;fastinglevelsofC-Peptideare5–10foldhigherthanthoseofInsulinowingtothelongerhalf-lifeofC-Peptide.TheliverdoesnotextractC-Peptidehowever;itisremovedfromthecirculationbydegradationinthekidneyswithafractionpassingoutunchangedinurine.HencetheurineC-PeptidelevelscorrelatewellwithfastingC-Peptidelevelsinserum
