Introduction:Rubellaisaherpesvirus.Generallyrubellaisconsideredamildadolescentdisease.However,amaternalinfectioncanbetransmittedthroughtheplacentatothefetus,causingcongenitalrubella.Congenitalrubellamayresultinchroniccardiacdisease,growthretardation,hepatosplenomegaly,malformationsandothersevereanomalies.Childrenbornasymptomaticmaydeveloptheseabnormalitieslaterinlife.Toreducetheriskofsuchseverecomplications,accurateSEROlogicalmethodsmustbeperformedtodeterminetheserologicstatusofchildbearing-agedwomen.Awomantestedtobenon-immunecanbeeducatedoninfectionanddifferentiaterubellafromotherexanthematousdiseases.Expectingwomenwithcurrentrubellainfectionshouldbecounseledontheconsequencesofcongenitalinfection.
Principleofthetest:PurifiedRubellaantigeniscoatedonthesurfaceofmicrowells.Dilutedserumisaddedtothewells,andtheRubellaIgM-specificantibody,ifpresent,bindstotheantigenduringincubation.Afterwashingthewellstoremoveunboundsample,antibodytohumanIgMconjugatedwithhorserADIshperoxidase(HRP)isaddedandincubatedat37Cfor30minutes.Unboundconjugateisremovedbyasubsequentwashingstep.AsolutionofTMBReagentisthenaddedtothemicrowells.Theenzymeconjugatecatalyticreactionisstoppedataspecifictime.TheintensityofthecolorgeneratedisproportionaltotheamountofIgM-specificantibodyinthesample.Theresultsarereadbyamicrowellreadercomparedinaparallelmannerwithcalibratorandcontrols.
LimitationoftheTest:1.ReliableandreproducIBLeresultswillbeobtainedwhentheassayprocedureiscarriedoutwithacompleteunderstandingofthepackageinsertinstructionsandwithadherencetogoodlaboratorypractice.
