Background:

ThevonWillebrandFactor(vWF)isalarge,multifunctionalglycoprotein,occupyingakeypositioninprimaryhemostasis.Ithasamultiplestructurewithseveralfunctions:

  • ItisthecarrierproteinforFactorVIIIinplasma;itformsacomplexandthusprotectsFactorVIIIfromearlyproteolyticdecomposition.
  • Itactsasamediatorforplateletaggregationbyattachingitselftoplateletmembranereceptors(GPIbandGPlIb/IIIa)followingpreviousplateletactivation.
  • Itplaysapartinprimaryhemostasisbyactingasamediatorbetweenadhesionedplateletsandthesubendothelium(lesionedvascularwall).ThevonWillebrandSyndrome(vWS)isthemostfrequentlyoccurringhemorrhagicdisease;itmaybehereditaryaswellasacquired,causedbyquantitativeorqualitativedefectsofthevWF.
