17Alpha-OHP(17alpha-hydroxyprogesteroneor17α-hydroxyprogesterone)isa steroidhormone producedintheadrenalglandandgonads.(1,2,3) Itissynthesizedfromprogesterone,anditservesprimarilyasaprecursorcompoundthatisconvertedintocortisolintheadrenalgland,orintoandrogenicandestrogenicsteroidhormonesinthegonads.(3,4)17α-OHPisroutinelyusedforthediagnosticassessmentof21-hydroxylasedeficiency,whichislinkedtocongenitaladrenalhyperplasia,impairedaldosteronesynthesis,andfatalsalt-wasting.(5,6,7,8,9) 17α-OHPexhibitsadiurnalrhythmwithhighervaluesinthemorninganddecreasingoverthedaytoanADIrintheevening.(10,11)17α-OHPenterssalivafrombloodviaintracellularmechanisms,andthereisexcellentcorrelationbetweensalivaandserumvalues.(12)​
