Introduction:AgroupofmucintypeglycoproteinSialosylLewisAntigens(SLA),suchasCA19-9andCA19-5,havecometoberecognizedascirculatingcancerassociatedantigensforgastrointestinalcancer.
CA19-9representsthemostimportantandbasiccarbohydratetumorMarker.TheimmunohistologicaldistributionofCA19-9intissuesisconsistentwiththequantitativedeterminationofhigherCA19-9concentrationsincancerthaninnormalorinflamedtissues.
RecentreportsindicatethattheserumCA19-9levelisfrequentlyelevatedintheserumofsubjectswithvariousgastrointestinalmalignancies,suchaspancreatic,colorectal,gastricandhepaticcarcinomas.TogetherwithCEA,elevatedCA19-9issuggestiveofgallbladderneoplasminthesettingofinflammatorygallbladderdisease.Thistumor-associatedantigenmayalsobeelevatedinsomenon-malignantconditions.ResearchstudiesdemonstratethatserumCA19-9valuesmayhaveutilityinmonitoringsubjectswiththeabove-mentioneddiagnosedmalignancies.IthasbeenshownthatapersistentelevationinserumCA19-9valuefollowingtreatmentmaybeindicativeofoccultmetastaticand/orresidualdisease.ApersistentlyrisingserumCA19-9valuemaybeassociatedwithprogressivemalignantdiseaseandpoortherapeuticresponse.AdecliningCA19-9valuemaybeindicativeofafavourableprognosisandgoodresponsetotreatment.

Principlesoftheassay:MicrotiterstripwellsareprecoatedwithmonoclonalantibodiesagainstCA19-9.DuringfirstincubationCA19-9insamplesandstandardsbindtotheimmobilisedantibodiesonthesurfaceofthemicrotiterwells.Unboundsubstanceswillberemovedbyasubsequentwashingstep.Duringsecondincubationspecificanti-IgGantibodiesconjugatedwithperoxidasebindtoCA19-9.Unboundconjugatewillberemovedbyasubsequentwashingstep.InathirdincubationtheenzymesubstrateTMBwillbeoxidizedresultinginabluecolour.Additionofsulphuricacid(stopsolution)stopstheenzymaticreactionandturnsthebluereactionproductintoyellow.Absorbanceismeasuredat450nm.TheamountofoxidizedTMBisproportionaltotheamountofCA19-9inthesample.

LimitationsoftheTest:Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.
