IntendedUse:TheToxocaracanisIgG-ELISAisintendedforthequalitativedeterminationofantibodiesagainstToxocaracaniscirculatinginhumanserumorplasma(citrate).

Introduction:Toxocaracanis(anascarid)isaparasiticnematode(roundworm)commonlyfoundintheintestineofdogs.Humansareparatenichostswhobecomeinfectedbyingestinginfectiveeggsincontaminatedsoil.Afteringestion,theeggsyieldlarvaethatpenetratetheintestinalwallandarecarriedbythecirculationtoawidevarietyoftissues(liver,heart,lungs,brain,muscle,eyes).Whilethelarvaedonotundergoanyfurtherdevelopmentinthesesites,theycancauseseverallocalreactionsthatarethebasisoftoxocariasis.
Inmostcases,Toxocarainfectionsarenotserious,andmanypeople,especiallyadultsinfectedbyasmallnumberoflarvae(immatureworms),maynotnoticeanysymptoms.Themostseverecasesarerare,butaremorelikelytooccurinyoungchildren,whooftenplayindirt,oreatdirtcontaminatedbydogstool.ThetwomainclinicalpresentationsoftoxocariasisareOcularLarvaMigrans(OLM),aneyediseasethatcancauseblindness(eachyearmorethan700peopleinfectedwithToxocaraexperiencepermanentpartiallossofvision),andVisceralLarvaMigrans(VLM),adiseasethatcausesswellingofancillarybody’sorgansorcentralnervoussystem.

Serology:DetectionofantibodiesbyELISA

Principlesoftheassay:ThequalitativeimmunoenzymaticdeterminationofantibodiesagainstToxocaracanisisbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.
MicrotiterstripwellsareprecoatedwithsyntheticToxocaracanisantigenstobindcorrespondingantibodiesofthespecimen.Afterwashingthewellstoremoveallunboundsamplematerialhorseradishperoxidase(HRP)labelledProteinAConjugateisadded.ThisconjugatebindstothecapturedToxocaracanisspecificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofToxocaracanisspecificantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.

StorageandStability:Thereagentsarestableuptotheexpirydatestatedonthelabelwhenstoredat2...8°C.

LimitationsoftheTest:CrossreactivitywithantibodiesagainstAscarislumbricoidesandSchistosomacannotbeexcluded.
Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasserologicaldata.
Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue.
